The breast is a mass of glandular, fibrous and fatty tissue. It is placed over the pectoral (chest) muscle and attached by ligaments called Cooper’s ligaments. Development of these parts of the breast begin at puberty and are dependent on hormones such as estrogen and progesterone. Women may have many changes in their breast during any point of their life. Some are dependent on their menstrual cycle. Others are linked to birth control pills, hormone replacement therapy, family history or lifestyle changes. Most breast conditions are not cancer. They arise from an abnormality in the normal development and ageing of breast tissue.

October is breast cancer awareness month. In a spirit of raising-awareness, we're giving you a two-part series on breast changes in - 1)Premenopausal women 2)Post-menopausal women

Part 1 - Premenopausal women

Breast pain

The most common complaint amongst younger women is pain linked to the menstrual cycle. This is called “Cyclical mastalgia”. It is almost always due to our hormones (estrogen and progesterone). As the pain does not always affect both the breasts, there might be other causes to pain in a single breast. This type of pain might begin at ovulation and last all though the menstrual cycle. Pain that involves both the breasts and is cyclical is generally not harmful. When the breast pain does not correlate with her menstrual cycle, it is called “Non-cyclical breast pain”. It is advisable to chart the breast pain to understand the link better. The pain might be minimal to so severe that you can’t wear tight clothes or move around freely.

When you experience such pain, wear a good bra with adequate support. Maintaining a healthy lifestyle but limiting alcohol, caffeine and increasing greens in your diet are helpful. Pain killers such as Paracetamol may be taken in recommended doses if the pain is severe. Vitamin E capsules and massaging breasts with EPO (Evening primrose oil) have shown some benefit in patients.

Fibrocystic breast changes

These changes are what you feel in your breast prior to your menstrual cycle, commonly in the third and fourth decades of life.  Breasts may feel heavy, sore or lumpy. These changes occur due to an overdrive of estrogen and a deficiency of progesterone. Although these are not cancerous, having certain pathologically proven lesions of the breast which fall under the “fibrocystic” category increase the risk of cancer three-fold.

Infections

Infections are common in breast feeding mothers, smokers and women with systemic illnesses. They may present with lumps, nipple discharge, pain or fever. It is very important to identify and treat these symptoms which may range from simple medical management to surgical treatment.

Cysts

Women who develop a cyst complain of a sudden development of a lump in the breast overnight. Cysts are fluid filled structures in the breast which occur due to a blocked duct or hormonal changes. They are most commonly benign. However, certain cysts may have a small component of a cancerous or precancerous lesion. They are generally seen in the third or the fourth decades of life.

They can be soft or hard. They may be felt by the hand or seen on imaging with a mammogram. They are generally observed or aspirated with a small needle.

Tumors (Benign)

Fibroadenomas (FA) are benign lumps that occur commonly in younger women, especially teens. They however can occur at any age group. They are mobile and slip when being felt. Women generally feel them when examining their breasts. If they are small and not growing, they maybe observed. However, if they are increasing in size or are symptomatic, simple removal is suggested. They very rarely turn cancerous (Complex fibroadenomas)

Phyllodes tumors are generally seen in the third and fourth decades of life and are prone to growing rapidly, changing the shape of the breast. The treatment is relatively complicated compared to a FA. Based on the nature of the tumor, radiotherapy might be required in certain cases and recurrences are common unless they are removed completely.

Breast Cancer

Currently, in India, the rates of cancer have increased in women between the ages of 25-40. The reasons for this might be lifestyle changes, similar to the West. If women under the age of 35 develop cancers, they tend to have a strong family history of ovarian or breast cancers. It is important to get counselled with a professional genetic counsellor.

Remember that ‘Breast Self-Examination” is the most important technique of identifying early breast diseases.  It has to be done every month. Make sure you reach out to a doctor if you find any abnormality in your breasts.

Bengaluru: Karnataka Deputy Chief Minister D K Shivakumar on Thursday backed Chief Minister Siddaramaiah over his claim that the BJP had offered Rs 50 crore each to 50 Congress MLAs in an attempt to "topple" the state government.

Addressing reporters here, Shivakumar, also the Congress state president, said, “The BJP indeed lured 50 Congress MLAs with Rs 50 crore each.”

He defended Siddaramaiah’s statement and said the Congress MLAs were briefed about the BJP’s alleged 'Operation Lotus', a term used to describe the BJP's attempts to destabilise ruling governments through horse-trading.

“Some of our MLAs informed the Chief Minister about this matter, and he, in turn, shared it with the media,” Shivakumar said.

At an event in Mysuru, Siddaramaiah reiterated the claim that "none of the Congress MLAs had accepted the offer".

He also accused the BJP of filing false cases against him in a bid to "remove him and overthrow his government".

The BJP has yet to respond to the allegations.

Bengaluru, Nov 22: For the second day running, the Karnataka BJP on Friday staged a statewide protest condemning the government’s alleged move to notify land of farmers as Waqf property.

The BJP staged a protest before the offices of Deputy Commissioners at district headquarters.

The BJP leaders are vehemently demanding that the state government cancel a 1974 Gazette notification in this regard.

The agitators are also demanding scrapping of the Waqf Board and the resignation of Chief Minister Siddaramaiah and Minister for Waqf and Housing Zameer Ahmad Khan.

The BJP MLAs, MLCs, MPs gathered in the premises of Freedom Park and staged a protest under the leadership of Leader of Opposition R. Ashoka and slammed the state government.

MLA T.S. Srivatsa led the protest in Mysuru and hundreds of party workers and farmers staged the protest under the leadership of former MP Pratap Simha in Kodagu.

Former MP Sumalatha Ambareesh led the agitation in Mandya.

This was the first time that Sumalatha took part in the party’s programme after the Lok Sabha elections.

State President B.Y. Vijayendra claimed, “The Congress government in Karnataka is issuing notices to farmers claiming the ownership of their lands to the Waqf Board and pushing them on the streets overnight.”

In the first week of December, three teams formed by the BJP will travel across the state and record the grievances of farmers.

“The state government is attempting to snatch away the lands belonging to temples as well,” Vijayendra alleged and added that the teams would comprise all senior leaders of the BJP.

Meanwhile, the police have taken Sri Ram Sena chief Pramod Muthalik into custody while staging a protest march to the office of Zameer Ahmad Khan in Bengaluru.

Muthalik along with Hindutva activists was planning to lay siege to Zameer’s office over the Waqf row.

The police stopped Muthalik and requested him to submit the memorandum by reaching the minister’s office in a vehicle. However, Muthalik refused to go with the police and continued his footmarch. The police took him into custody following arguments.

Beirut: The Israeli army on Tuesday continued to launch attacks against civilians in Lebanon, targeting them in several areas without prior evacuation warnings.

However, 13 airstrikes on Beirut’s southern suburbs in the space of only three hours were preceded by evacuation warnings.

The attacks caused no injuries but resulted in widespread destruction of residential buildings and commercial, medical and educational centers.

The airstrikes in southern Lebanon and Bekaa region, reaching Akkar in Lebanon’s far north, erased any hope of a near-term ceasefire settlement.

The strikes were accompanied by an announcement on Israel’s Channel 14 that “the Israeli army has expanded its operations in southern Lebanon to areas it had not reached since the beginning of the ground operation.”

About 50 days have passed since Israel intensified its hostile operations in Lebanon targeting Hezbollah. The death toll from these confrontations and attacks has passed 3,200, with more than 14,000 wounded.

For the first time, an airstrike targeted a mountainous area between Baalchmay and Aabadiyeh on the road leading to Aley, destroying a building housing displaced people.

The mayor of Baalchmay, Adham Al-Danaf, confirmed that “the airstrike targeted a residential building in the Dhour Aabadiyeh area.”

The initial toll from the Ministry of Health showed “five people killed and two injured.”

The raids that targeted Beirut’s southern suburbs for the first time in the morning, unlike nightly raids before, caused huge destruction. Those who evacuated their homes after Israeli warnings, used their phones to record the collapse of empty buildings in Sfeir, Haret Hreik, Bir Al-Abed, Mrayjeh, Laylaki and Hadath.

Israeli warplanes also targeted Tyre, where a strike on a building killed three people and injured many others, while a raid on Tefahta killed a man identified as Kifah Khalil and his family.

Attacks were widespread, with Yater and Zebqine subject to artillery shelling, a civilian being killed in Hermel, and further attacks on Bouday and an area between the towns of Srifa and Arsoun.

A raid on the town of Siddiqin killed two people and injured several others, while an attack on the Mechref farm led to one fatality and multiple injuries.

The search for those missing after an Israeli raid on the town of Ain Yaacoub in Akkar, in the northernmost part of Lebanon, continued until dawn.

During the operation, 14 bodies were retrieved, identified as those of residents displaced from the town of Arabsalim in the Iqlim Al-Tuffah area of the south, along with members of a Syrian family, a mother and three of her children. Additionally, there were 10 people in critical condition.

The targeted residence belongs to a Lebanese citizen, Hussein Hashim, who is reported to be a member of the Syrian Social Nationalist Party.

An airstrike on the town of Saksakiyeh in the Sidon region on Monday night resulted in yet another tragedy.

It appeared that the intended target was the Shoumer family, who just days before lost Hussein Amin Shoumer and his two sisters in a drone strike near Al-Awali River.

Israeli army spokesperson Avichay Adraee issued additional evacuation warnings for towns in the southern region along the Litani River, which, according to estimates from the mayors, are currently 90 percent uninhabited.

In the meantime, Hezbollah announced its continued efforts to “combat the intrusions of Israeli forces and to strike military installations and towns in the north.”

Hezbollah said in a statement that it confronted “an Israeli Hermes 450 drone in the airspace of Nabatieh and forced it to leave Lebanese airspace.”

The party also announced that it targeted “Kfar Blum settlement with a rocket salvo.”

On the Israeli side, air raid sirens sounded in areas of Upper and Western Galilee and in the town of Kiryat Shmona and its surroundings.

The Israeli army confirmed that “a drone exploded in Nesher, east of Haifa, without activating the air raid sirens,” and that “a drone launched from Lebanon crashed into a school in Gesher HaZiv, north of Nahariya.”

Israel’s Channel 13 reported the Israeli military’s assessment regarding Hezbollah’s military strength, claiming that the group currently possesses approximately 100 precision missiles, thousands of artillery shells, and hundreds of rockets. Additionally, it was highlighted that “there are around 200 Lebanese towns that remain unvisited.”
